If it is on the floor he pees on it. For example if the kids leave one
piece of clothing on the floor he pees on it, if I have any rugs down he
pees on it. He just ruined my area rug. He has been doing this for close
to 2 years now. My vet it at a loss for why. Any help would be great.
Also he does use his litter box as well.

Bless you for still having him!  You are a good soul!  All I can think of
is, #1: Has he been neutered?  #2: Have you tried Feliway?

Well he is part of the family, if one of my children has a dehavoir problem
I would not get rid of them LOL. Yes he is fixed and I don't know what
Feliway is.

Seems like your cat has associated cloth to be the same as cat litter.
Maybe you can provide an appropriate place for him to act out this
behaviour.  Try lining a litter box with an old towel you can wash daily.
He may perfer cloth for #1 and litter for #2.  If he takes to this you may
need to keep all cloth off the floor for a while until the new behaviour is
reinforced.

Feliway is a product that mimics the cat's pheromones and reduces marking
behaviors.  It has a claming effect on most cats and makes then less
territorial.  It just makes things smell to the cat, as if all's right with
the world, though it's odorless to humans.  It comes in a spray as well as a
plug-in diffuser, if I'm not mistaken.

So long as you've ruled out health issues, you might just try a good enzyme
cleaning of everything peed on, followed by application of Feliway to the
entire area.

it sounds to me like it might be a behavioral issue! That is absolutely the
hardest thing to break.  I wish you the best of luck!

What tests did the vet do?  Was an ultrasound done?  Certain types of
bladder stones that make urination very painful can only be detected by an
ultrasound.  If that hasn't been done, I'd have the vet do one.  If that
doesn't solve the problem, I'd consider getting a second opinion.

Are you cleaning everything he pees on with an enzymatic cleaner like Anti
Icky Poo or Nature's Miracle?  How many litterboxes does he have?  Some
cats may not want to urinate and defecate in the same box - you might try
adding another box, or using Dr. Elsey's Cat Attract litter
(www.preciouscat.com).
