Cat peeing outside box

I'm at my wits end with this.  I had a bad experience with a girl cat
before and now I think it's happening again.  I had two males and a
female. From very early on she urinated all around the house.  We
tried EVERYTHING and after a couple of years, we had to get rid of
her.  Luckily my aging step-father took her in and being alone seemed
to work for her. She stopped peeing in the house.  A couple years
later I took in a stray and since then (last February) things have
been fine. She's a little strange (only drinks from running water) but
she's adapted to household living quite well. Well now one of the cats
is urinating outside of the litter box. I am not going through this
again as it nearly ruined my former house the last time this happened
with the previous female.  The boxes are cleaned twice a day (I have
two of them) and she's been going in it for months. I'm thinking of
separating the cats to make sure it's the female.  Does anyone have
suggestions about this? The last time it happened my cat was put on
all kinds of meds and I spent hundreds of dollars checking for urinary
track infections etc.  I really don't want to go through this again
with the new stray but I suppose I have very little choice.  I'm just
reaching out before I go any further.

A friend of mine had the same problem she had to put the cat outside
like a dog on leash because the cat was too "clean" to put his paws in
the "dirty" litter.
