My tomcat Diego roughly 18 months old has always had small cuts in his ears
and the inner ear (when I say inner I do not mean right in) seems tender and
he gets these scabs which are small.  He is a very clean cat and he has a
check up when he had his boosters but I forgot to ask vet, they come and go.
Is this anything to worry about.  My other cat who is 4 years 6 months and
female has never had this problem.

Call the vet and ask.

Pretty common in my experience.
Watch the cat clean it's ears, then you'll see how
they get scratched.

Yes I see what you mean, it makes sense.

The small female cat I have LouLou has no such problems though.

I would try the vet on this one, MD.  A correct diagnosis and a
prescription for his specific problem will likely clear it up quickly,
with a minimum of time, effort, and expenditure.  Over-the counter
products may or may not do him any good at all, as you will be "shooting
on the dark" without knowing what is causing the problem.

I would recommend the vet however someone did post a site where you could
buy ear drops.  are you in the UK or US?

check out this site if you want to avoid a trip to the vet.  however some
lines are prescription only.  if you do go to the vets you are now able to
ask for a prescription rather than the product be dispensed by the vet.  you
could use this site to source the ear drops.

my black cat had ear mites last year - must admit it was so obvious there
was a problem, he kept shaking his head and scratching.  it must have been
really irritating for him.  the ear drops prescribed solved the problem

my kitten is five months old and she has a bald ear.  I took her to vet and
she has dirty ears (not sure if the vet was politely saying she had bugs!!)
once again we were prescribed ear drops and paid £8.

if Diego went to the vet, I am surprised that the vet didn't notice it on
their own.  Checking the ears is pretty standard routine exam.  So I would
tend to worry about that, if he had them before that and the vet didn't
notice.

Mites produce debris that looks a lot like coffee grounds.  Pretty gross.
He could also have a yeast overgrowth, which produces a more waxy type
debris.

Could also be fleas, and the ears are just getting caught in the crossfire.
Could be a combination of everything.

Diego's ears have cleared up again now but he does seem to discharge a fair
amount of ear wax.  Where my cats sleep I always put white linen for them to
sleep on so I can check if they pick anything up.  It takes regular washing
but it is amazing the amount of dirt that a cat brings inside the house,
when to look at them they appear so clean.  There is no sign of fleas but
wax on linen.
