My almost 20 year old cat is defecating all over the house and back yard.
Does this mean she is incontinent? or senile or what.?

It means that she has become old and ill, and that she needs to see a vet
pretty soon.
Congrats that you managed to have a cat that became so old, but now she
needs your help.

Take her to a vet to see what is wrong. Your cat could be very sick.

I took her in today and it appears that she has a case of alzimers/ aside
that she appears to be in decent health for a cat being born in 1986.
She purz very loudly when she gets held and is affectionate towards. And the
vet prescribed her (methimazole) for the behavior problems a memory
stimulator and diagnosed her with (Hyperthyroidism) Also recommended a
slight change in her diet for the uncontrolled liquid discharges. Even
though she goes any where she wants, she seems to target frequently a cretin
area where herself and my late dog urinated and defecated years ago. We cant
seem to get the smell out of that section of the carpet. And the funny
thing is that my cat rolls around in that section and scratches it allot.

I would say she is sick and you should take her to a Vet right away.

It's not the end! please don't say that... she has been with me for 20
years. She is family to me and all I have. I took her in this morning and
the vet gave her a shot and inserted his finger in her rectum and did
something only a vet could understand. I turned my head as she screamed.
She is now doing better. She had a normal bowel movement for the first time
in almost a week. And she went in the litter box. Status quo as of now but
im going to look in to alternative health as well. The vet contributes this
all to a old age. I will do my best to keep her alive as long as I can, as I
love her as a human.
