Early Renal Detection

Heska released thier diagnostic test kit for Early Renal Detection in
cats. The  previously available testing method of analyzing blood for
BUN Creat etc. only detected renal failure when  >70% of the kidney
was already destroyed and the disease was in terminal progressive
stage. This new process catches kidney disease far earlier. We've had
the discussion on this NG a couple times regarding kidney failure. In
thier testing for the diagnostic they had some interesting results.

1. 25% of 1,200 cats tested were positive for renal failure.

2. Of 611 apparently healthy cats, 13.7% tested positive.

3. Of older cats ages 12-15 35.6% of cats tested positive.

The test can have false positives if there is:
a) concurrent inflammatory disease (dental, FLUTD, cholangiohepatitis,
IBD)
b) concurrent infectious disease (FeLV, FIV, FIP, heartworm,
erhlichiosis)
c) metabolic disease (diabetes, hypertension, hyperthyroidism)
d) neoplasia

This is good news.  I'm going to mention it to my vet.  How does this
work to get the word out to vets?  Is there some sort of widespread
newsletter they get?  ;)

Demelza's now 11 (so I now have a full blood panel done for her annual
exams) & it'll be interesting to hear what my vet(s) say(s) about this -
their take on how useful it is at this stage of the game.

  Heska has a small sales force and they sell through several drug
distributors. I'm sure your vet will be getting information fairly
soon. Heska had a booth at the Central Veterinary Conference last week
and launched the product there. One of the speakers on renal failure
talked about the test, but didn't spend a lot of time on it as he had
only used it for a short time. Hill's felt it was important enough
when they came out with the canine version that Hill's gave away
several thousand kits across the country. Obviously the thinking was
that if more renal disease was discovered, more k/d would be sold.
That won't be happening with the feline version, so your vet will have
to buy the kit.
 I would expect there will be articles and studies published in
various vet journals as people have a chance to work with the kit over
the next year or two. I think the kit is a terrific plus. While it
does have the problem of false positives - infection etc, detecting
those problems is also beneficial. When the canine test hit the number
of undetected non-renal medical problems was very high - that benefits
the pet by alerting the vet to a hidden non renal failure medical
problem that might never have been discovered otherwise.

This is really good news, Steve, thanks.  Is Hill's not doing the same
with the feline version because giving away the canine version didn't
result in more k/d being sold or for some other reason?

 In a way yes, about 80% of the kits we gave away are still sitting
on the shelf in the hospital a year later. Veterinarians are timid
about adding another $20-30 to a geriatric profile. It's really too
bad as there is no question that if vets started using these routinely
for cats and dogs they would discover renal disease much earlier and
have a much more successful outcome.

If this really works, this sounds like a wonderful tool.  Will this be
available in vet's offices soon?  Is this something that should be
part of a normal check-up, or routine bloodwork?  This sounds like it
has exciting possibilities, and I will be very interested to see what
happens.
