enlarged kidneys

Hello,
My 15 year old cat started losing her appetite suddenly and
consequently weight.  She stopped jumping on the windowsills and hid
most of the day.  I took her to the vets and they said her CBC showed
that just her liver numbers was a bit elevated.  All else OK.  She
still didn't want to eat but was peeing normally.  Back to the vet
where they did an ultrasound which they said showed very enlarged
kidneys from a probable cancer.  We also did another CBC which showed
her kidneys were functioning fine.  I was sent home with Prednisone
which has helped her show an interest in food. She is still urinating
and pooping normally. The vet sounded hopeless and due to her age they
didn't do a biopsy but is very encouraged with her new interest in
eating.  I am more concerned about the large kidneys being caused by
something other than cancer.  She also has a heart murmur and
irregular beat.  She had been palpitated frequently on these visits
and no odd lumps were found anywhere.  Any ideas out there?  I am
trying to see what the different feline boards might have for thoughts
on this subject.  I will probably get a second opinion.  Thanks.

have a three year old male who has weight loss, and enlarged kidneys. The
vet did not do lab work, but instead said it looked like cancer and we
should put the cat to sleep. Would a blockage cause the kidneys to be
enlarged? One is the size of a the vet's hand and the other is also
enlarged. We don't want to put this cat to sleep if there is a chance
something can be done. . Can you respond as quickly as possible as our appt
is tonight and we are anxious to do the right thing, but do not want to put
our cat to sleep if there is hope for him. Thank you.
THE ANSWER

Sorry to hear about your cat, and for the delay in getting back to you (I
have been away from the office). It is very difficult for me to comment
about a specific diagnosis by email, where one cannot actually examine the
cat and will have to defer to your vets clinical judgment about the
prognosis. Cancer would be one thing to consider (and probably related to
FeLV if it is, and if you have other cats they should be tested). There are
other conditions that could cause enlarged kidneys, including any
obstruction that has been present a long time, chronic kidney infections,
and also congenital diseases of the kidneys (such as polycystic kidney
disease). It is impossible to say which of these is present without any
tests, but assuming that you cat is showing signs of renal failure, the
prognosis would be very guarded for any of these conditions at the stage
where the kidneys were so markedly enlarged
