I have a friend with an 11 year old female Calico cat. A few years
ago, after another cat entered the household, she bagan to
occasionally pee and defecate out side of the liter box (on the bed,
couch, etc). Since then, there was a time when she was again the only
cat, but she continued this behavior. Now she's one of two cats again.
We're not sure if this is behavioral or not, or, if it's more
physical, what it could be or what if anything can be done. She really
does not want to give the cat up, but she doesn't know what to do any
more. She's going to the vet next week to have her checked for kidney
infection. She's tried different litter, more attention, 2 litter
boxes, and asked many friends about this, but no real solutions have
come up. Any suggestions? She does this when the owner is there as
well as when she's absent.

Sounds medical to me. If there is no infection, she may have intersitital
cystitis and need anti-depressant meds for pain (while urinating).
