please ...more purrs for my mom

My mom fell and broke her hip in late July and had her hip replaced. She was
in the hospital for 5 days and then they moved her to the rehab floor.
Since her surgery she's had some difficulties, especially with getting
sufficient oxygen -- turned out she has "mild" congestive heart failure
(from "mild" leakage in 3 heart valves) and has had fluid in her lungs.
They also said she has "moderate" COPD, which I think stands for chronic
obstrutive pulmonary disease, which we didn't know before either.

She had been improving, was off oxygen, and was due to be moved  from the
intensive rehab center to a skilled nursing facility on Friday.  Found out
today she's had several episodes of bleeding from her blood thinner
injection site; they controlled that but then she was complaining of a lot
of pressure under her ribcage.

I called my friend who is a cardiac nurse and  basically got the scoop from
her on what to ask the rehab nurse to do (which included calling the doctor
and requesting they do an EKG) and so have been following EKG and troponin
tests this evening.  The thought is possible heart attack.  If you have
purrs to spare, please send them on to her.

Purrs.

She may be on such treatment already, but she should be cautiously on a
beta blocker, which improves survival in heart attack and chronic heart
failure, but causes airway problems in asthmatics. An ACE inhibitor is
also appropriate and safe for the airway. Low-dose aspirin is also
probably appropriate, but with careful monitoring of blood coagulation
and any brochospasm.  Diuretics may be useful in getting rid of the
fluid.

Troponins and EKG are useful, but I'd probably want a CK-MB blood test
as well as electrolytes and possibly blood gasses. If there's a question
of the anticoagulant (blood thinner), she should have a coagulation
panel, which consists of the prothrombin time (PT), partial
thromboplastin time (PTT), and the computed Internationalized Normal
Ratio (INR).  High INR probably means she should get Vitamin K.

Just so you know ... generally speaking, the hip (femur/thigh bone)
fractures first which in turn causes the patient to lose balance and fall
down.   I have been through this with my father who had a partial hip
replacement at age 95.  Repeated loss of balance and falling is a telltail
sign of advancing dementia which is the body's natural, progessive shut down
process.   I am sorry you have to go through this.

Thank you all sooooo much for the purrs and everything; the lab test results
for the troponin came back negative which was wonderful and by morning the
pressure under her ribs had gone and by early afternoon today the bleeding
had stopped..  In fact, tonight, she sounded sooo much improved compared to
last night that I felt an overwhelming sense of relief. As you can imagine,
I didn't sleep much last night running all kinds of scenarios through my
head. So huge THANK YOUs to you all and your purrkids for the support; it
made the difference!

Will post updates to questions re cats and kids to those who asked as soon
as I get a chance to catch my breath.  Thank you all for caring.

That is good news.  The power of purrs and prayers is awesome.

It sounds a lot like what my 89 year old aunt went through last week.
She was hospitalized in ICU last Thursday with congestive heart
failure and was unresponsive.  Her BPN was 1800 and should have been
below 100.  The doctor said that her heart,  lungs and kidneys were
shutting down.   By Friday morning she was sitting up and talking, but
was passing blood.  She had a colonoscopy Saturday morning which
showed some polyps and a few small veins, but nothing that should have
caused the bleeding.  They decided that her blood thinner level was
too high and was causing the bleeding.  Monday morning she was well
enough to be released.

I live thousands of miles from my mom and only keep in touch by phone.
