Almost certainly, a blood vessel either wasn't sealed/cauterized/sutured
closed, or something made it start leaking. Elsewhere in the body, it
might have not shown much effect, but the problem is that the scrotum is
"out there", with limited tubing into the rest of the body.
*HUMAN GROSS WARNING*
I remember some impressive photographs, in a medical journal, of a man
who seemed to be sitting with a purple ummm...cantaloupe in his lap. He
had had a blood leak from a vasectomy, which could only drain into the
scrotum.
Open the scrotum, let it drain, find the bleeder, cauterize or tie it
off, stitch the scrotum closed.
Excessive fluid in the scrotum, in humans, can lead to a rare and
dangerous form of compartment syndrome, Fournier's Gangrene.

*** ANOTHER SQUEEMISH FACTOR WARNING***
"Free air" in the body---such as that caused by a collapsed lung-- can cause
the same kind of effect. I saw this once years ago when I was working in an
ER as an x-ray tech student. A car accident victim had the same kind of huge
swelling and a doctor standing next to me muttered that that was the largest
scrotum he had ever seen. I asked him later about it, and he said it can be a
common occurance for car crash victims, and even after surgeries that involve
forcing bottled air into the trunk of the body to make room for the
instruments to move around freely. Free air moves to and collects in
whatever place in the body it can find that expands---and loose skin can
expand ALOT. I never did find out what they did to a.......um....deal with
the problem for the poor fellow, though.
Back to topic----I am glad to hear the kitten got the help he needed.
