Please could HRFL Tiger have some purrs and prayers? We've just got
back from the vet and I am worried sick. He's been a bit "OFF" for the
last few weeks, nothing particular, just a bit more clingy than normal
and a bit quieter, so tonight off to TED we went.
The vet listened to his heart as he has a 3/6 - 4/6 heart murmer,
hypertrophic cardiomyopathy and Tricuspid Valve displaysia. He only
listened briefly, and then said, "I don't want to concern you, but I'd
like Tiger to have an ultrasound in the morning". Not next week, or the
week after (the waiting list is 10 days), but instead he's been pushed
to the top of tomorrows list as an emergency.
The vet in question is not known for dramatics or bullsh*t, and if
anything he tends to take a more relaxed approach to treatment. He's a
Welsh farm vet who considers everything twice - especially where an
owner is paying. For him to react so decisively like this to a cat just
being "OFF", has me very worried. It's not what he does.
To put it mildly, I have never been more afraid in my life. And I've
been in some very scary situations. Please pray to whatever God you
believe in that I'm over-reacting and being an over anxious mommy
blowing things out of proportion and that Tiger is going to be OK.
I'll update you when I know more tomorrow.

Purrs and purr-ayers are on the way, even as I type.
And look at the bright side -- an appointment tomorrow means you'll have
answers that much sooner. It means you won't have to wait days to even get
the ulrasound DONE, never mind wait for results. I've always found it
easier to deal with actual information, regardless of what it is, than the
process of WAITING for information .
So here's hoping that the conclusions the vet draws indicate that anything
that shows up on the test is easily treatable.

The ultrasound is appropriate to differentiate between different forms
of cardiomyopathy. It will also determine the pumping efficiency of the
heart, or ejection fraction. I don't know normal values for cats; a
well-conditioned human is 50-65%, and we start to diagnose what is
called congestive heart failure (which does NOT mean the heart has
"failed") when the level is below 35-30%. Again, don't take these
numbers as meaningful for cats -- I don't know the normal range for a
cat. Still, you might want to ask about ejection fraction as a measure
of severity.
There are quite a few drugs that can help, and people do lead normal
lives. Weight and salt restriction may be important. In humans, you
would normally start with some combination of diuretics, beta-blockers,
and possibly digitalis. Additional drugs also
There are several invasive procedures used in humans, but I would
suspect most are impractical outside a major veterinary school. These
include catheter ablation of some parts of the heart (an alternative to
open heart surgery) a new type of pacemaker, and valvular surgery.
Realistically, the only one that I suspect would be feasible for cats is
ablation.
Again, lots of humans lead long lives with heart failure. They may be
restricted as to exercise, and you MUST be aggressive in getting
antibiotic treatment if there is any suggestion of infection. You may
have heard of people that need antibiotics before dental work -- same
thing here. If bacteria are dislodged by dental work or other activity,
prior medication with antibiotics prevents the germs from lodging in the
damaged valve, which could be very serious.

Thanks for this Howard - it helps to have it explained a bit. If push
comes to shove and the vet thinks it prudent, we're only about 35/40
miles away from Bristol University vet school and the FAB Feline
research center, so if we need to a feline cardiologist will be our
next port of call.
