Purrs for my Mom, Please

Just got a call informing me my mom is in the hospital.  Apparently there is
some sort of internal bleeding, no further information.  I talked with her
and she says she feels fine.  Well maybe so, but if you were fine you
wouldn't be in the hospital would you?  Oh, I guess not. (sigh)

While we're at it let's throw in some purrs for my father who can't manage
anything without her.  Thanks, folks.

Not really; I spoke with her this morning and I'm very ticked off.
Yesterday afternoon when she was admitted mentioned to the doctor she'd had
a similar problem about 3 years ago.  (Sorry to be gross but we're talking
rectal bleeding here.)  So he said he'd go pull her old chart.  She hasn't
seen him since!

Then she asked a nurse this morning about breakfast and was told she can't
eat "until after her test".  But no one seemed to know what kind of test or
when the test would take place.  She's seen no other medical personnel at
all!  Come on, folks, we're talking about an 81 year old woman bleeding
internally from some unknown cause and source.  WTF are they doing?!

And my father is distraught.  He's only half there mentally under the best
of circumstances.  He showed up early this morning, clearly upset, which
only made Mom worry.  He shouldn't be driving.  And he can't take care of
himself; he certainly can't cook.  He can make a sandwich, heat up a tin of
soup, that's about it.  But will he?

I hope his friend Ralph and his wife are looking out for him, not that it's
their job to do so.  I can't get down there due to monetary constraints.  My
brother Scott just got back on Sunday from spending a week there; he doesn't
have any more vacation time.  Why couldn't this have happened when he was
still there?  It's all very frustrating.

We have a few morons on another newsgroup who keep arguing
that the U.S. has "the best medical care in the world"!
(That may be true if you are among the wealthy, who can make
whatever arrangements they like, and are not dependent upon
insurance to pay for it.)  At least anywhere with
"universal" health care, emergency cases get prompt
attention - however long you must wait for hip replacements
or other "elective" surgery (meaning non-life-threatening).

Trust me, due to career decisions Jill's father made long ago, they can get
the best medical care available in the world.  Virtually without cost.

However that does not mean those employed by the medical establishment are
any better than anywhere else.

I'm quite sure, especially after some of Lesley's posts, prompt attention
for emergency cases stops abruptly as soon an one is admitted into care.

Jill, please accept purrs and prayers from our gang.  It has to be horribly
frustrating to be on the other end of a phone where you can't find out what
is really going on.  And can't possible hope to be any kind of advocate for
you mother in these circumstances.  It surely sounds like she needs one.

I do hope someone is keeping an eye out for your father.  You don't need
that extra worry right now and neither does your mother.

I have no idea why they didn't go to the base hospital.  I guess Dad just
took her to the closest facility which is in the small town of Beaufort, SC.

True, but they've always gotten very good care at the base hospital on
Parris Island.

Thank you very much (all of you).  I'm going to call now and see if she
knows any more than she did this morning.  I'm also going to make sure she
signs HIPPA forms to authorize release of medical information to my brother
and myself.  If *they* won't tell her what's going on (she's a little timid)
by gosh I'll make someone tell ME!

It is so confusing to the older folks. They need an advocate. Someone
to keep up with the
details, explain things to them, make sure they take their meds, etc.
I hope someone
is able to help them out.

Exactly right!  It isn't fair, but elderly patients receive *much* better
care if they have active (*persistent*) advocates who work on their behalf.
The exception would be those who are healthy and competent enough to act as
their own advocates, but it seems to me that the need for active
intervention rises diametricaly as people become less able to take actions
for themselves.  My mother is 91.  She lived with me and I was her sole
caretaker for 5 years.  I had to place her in a nursing home 5-1/2 years ago
when she became completely bedridden.  That was possibly the most painful
decision I have ever made.  I accept the fact that I could not give her the
same level care as a nursing home can, but I have also been very active on
her behalf.  I have made demands on many occasions where she would have
received inferior care if I had not noticed problems that staff either did
not notice or ignored.  One would think the same thing would not occur in a
hospital.  Unfortunately, it has been just as bad or worse.  A few months
ago, she was incorrectly listed as diabetic (in a local hospital)!  This was
"after hours" when another doctor was on call.  I discovered the problem,
had to argue vigorously that  discontinue must be placed on insulin until
her doctor was consulted, and did everything possible to make sure that the
incorrect diagnosis could not appear at some later date in her records.  I
was assured that there was "no way" that could happen.  It did, though!  She
was sent back to the nursing home with the incorrect diagnosis and a
prescription for insulin.  I discovered that *after* she had been given an
injection of insulin.  Moreover, when I followed the trail later, I
discovered that the director of nursing at the nursing home had contacted
the hospital because it made no sense that she was not diabetic (frequent,
routine blood tests) when she was sent to the hospital but was diabetic when
she was discharged a week later.  She was told that there was no mistake and
insulin must be adminsitered.  Fortunately, no harm was done, but this could
have been catastropic.

FYI:  I did follow up on this, going "through hoops" with the nursing home,
hospital, and her doctor.  As a direct result, the hospital has instituted a
change in procedure and held some additional training sessions.  By
coincidence, I received a certified letter yesterday from the Chief Qualify
Officer outlining steps in the review process that have been instituted by
the Quality/Peer Review Committee.  I hope this will help others who may not
be in a position be as insistent (and possibly as stubborn) as I sometimes
am.

That is a very good point, Mary. The elderly *need* an advocate, in
person. Someone who
has their best interests in mind. My father can't even remember what

after we leave. I would not dream of leaving him in the hospital
without me or my sister there,
no matter what.  And your mother is proof that the patients *with* a
family member
present sometimes (I think *most* times) ... get better care.
He says he doesn't want us there sometimes, too. I honestly don't
think he means it. He's always
very glad to have us keep up with the mountain of details that goes
along with it.  I think
sometimes he just feels bad that he is "putting us out", being a
burden.  You have to let
them know *they* are your priority at the moment.

Can I offer my personal thanks for your persistence?  Medical procedure and
errors are such a bear to take on.  Bless anyone who does it.

My mom had a close friend who was a nursing teacher for many years.  She
said teaching procedures and science wasn't the hard part.  And for the most
part the students were caring and dedicated.

But that it was extremely difficult to get across that errors must be
reported.  Always.  No matter who made them.  And that those reports must go
up the entire chain of responsibility.  That solution and prevention could
only happen that way.

She said it was particular hard when she lived and taught in Egypt.  These
students had been raised in a culture where they wanted to please.  When you
asked them a question, the idea of truth or lie didn't even occur to them.
Only what answer will please the person asking the question.  Plays havoc
with quality control.

But it isn't all that different in the US anymore.  Honesty and integrity
aren't quite the core values you might with.  Plenty of kids think the
George Washington and the cherry tree story is just plain dumb.

Funny thing is most all of us expect everyone else to be honest and careful.
But want a little wiggle room of our own.

I talked with Mom about signing HIPPA paperwork to allow them to tell us
(outside of the banalities) her medical status.  She said she'd think about
it.  What there is to think about I don't know.  But we're all 13 hours away
from where they live.  I can rant over the phone all I want but without a
HIPPA release they can't legally tell me a damn thing.  She's home now.  Dad
called to tell me he was making clam chowder for her. (Okay, he opened up a
can ... it's a start.)  This episode scared the hell out of him.  He
couldn't make it on his own.

I am in an entirely different set of circumstances because my mother is no
longer able to make decisions on her own.  Fortunately, she signed a living
will and made provisions for me to serve as her durable power of attorney
(both medical and financial) at a time when she was still competent.  She
also signed a DNR (do not resuscitate) order some years ago.  All of that
involves some painful decisions, but she eased many of the painful decisions
for her family by leaving clear directives while she was still able to do
so.  I have done the same thing myself.  These are vital decisions that too
many of us avoid until it is too late to make our own decisions.  (I realize
this isn't what you were asking abou -- I once again extended this to an
extent where it may be piggybacking, but I do think this is something
important that all of us should consider.  I also made provisionsin my will
for the care of my cats.)

We can't avoid it eventually, and we shouldn't, anyway.  They looked after
us when we were small and in the end they they will need us like we did them
once.
It's the least we can do, really.
