My male cat, Jake, has gotten into the habit of pooping right outside
the litterbox. He pees in the box, but does his other business right
in front of the box.
I figure it's not a medical issue since he does use the box.
Any suggestions?

The only thing I can think of is a bigger box, he may be misjudging the
size.

My Sarrasine had exactly the same problem until one day by accident I
tipped what I thought was too much litter in the box. Problem solved.
She just likes to seriously bury the evidence!

It is a fairly large box. I used to have problems with him hanging his
butt over the edge to pee. I think he just got into the habit of doing
it there.
I do try to keep the litter swept up from outside the box, thinking
that may be encouraging him to do it outside the box.

Bad, bad joke, but what you said reminded me of the Taco Bell commercial;
"Think Outside The Box". I will never be able to watch that commercial and
not think about a litterbox again. :-/

LOL! Nikki started "thinking outside the box" in her last years. I think
she just didn't notice that her backside was hanging over the edge. And
how could I be mad at her when she had all four paws firmly planted
inside the box? I just put a layer of newspaper under the box.

Bigger box? I use those rubbermaid storage bins that go under the bed. Nice
and roomy.

I used those things before, too, but I need the lidded litterboxes. Chloe
digs to China, I swear, and litter goes EVERYWHERE - even in a covered box!

Fin was a serious scratcher, and anywhere the litter box was quickly
became a desert. A covered litter box solved most of that problem.
They make covered litter boxes really large today, so size won't be a
problem.

One of mine does that too, I'm not even sure which one. Sometimes he or she
will use the box for a few days and then suddenly I find poo next to the box
again.
Makes no difference if the box has just been cleaned or not.
MLB, the boxes here are large enough for a whole cat.

Melisande does that occasionally, too - even immediately
after it's been scooped out (and the box is certainly large
enoough). She will scratch in it enthusiastically, then
position herself so the crucial portion of her anatomy is
aimed just outside the rim of the box. (If I happen to be
there at the same time, I can give her a slight nudge before
she gets to work, but otherwise.....) I don't have room for
a covered box where I live now, but next time I move I think
I'll try one.

I think the box is big enough. It's a covered box 'cause he used to
hang his butt over the side. I've seen him squat outside to dirty so
it's not 'cause he's hanging over the side.
I may try one of those plastic garment containers, see if that makes a
difference.
Someone mentioned about him "thinking outside the box". I almost put
that in my post she was referring to!
